Claims Documents
Basic Requirements
  • Medical Insurance - Hospitalization & Surgical Claim Form (C13)
    • Part I - completed and signed by Patient
    • Part II - completed, signed and stamped by Attending Physician (Doctor)
  • Original Statement of Charges / Account and Original Hospital Receipts
  • HKID Card / Passport copy of both Policyowner and Patient (if you have never provided clear copy(ies) of such document(s) to us before)

Additional Requirements (if applicable)
  • Diagnostic / Laboratory Reports (if Diagnostic / Laboratory Test done)
  • Referral Letter from Attending Physician (Doctor) (if attended by Specialist during the confinement)
  • Copy of Police Report / Traffic Accident Report / Police Statement (if involved in Traffic Accident)
  • Student Identity Card copy (if patient is a student at age of 18 to 25)
  • Certified true copy of compensation breakdown from other insurance company(ies) (if claim from other insurance company(ies))
  • Copy of Home Visit Permit and Daily Hospitalization Record (if hospitalized in the Mainland China)
  • Copy of Hospital Discharge Summary / Slip or Sick Leave / Medical Certificate with Diagnosis (if hospitalized in Government Hospital in Hong Kong)

After making a claim, you may login to our Customer Website, and check the claim status or view past claim records (up to the past 5 years) under “Individual Insurance > My Claims” section.
