Claims Documents
Basic Requirements
  • Group Hospitalization & Surgical Claim Form
    • Part I (Member information) - completed and signed by Patient / Employee
    • Part II (Doctor Statement) - completed, signed and stamped by Attending Physician (Doctor). It should have stated the date of treatment / hospitalization and diagnosis. If operation was done, please advise the operation name. (All items must be filled *#).
  • Statement of Charges / Account
  • Original Hospital Receipts

Additional Requirements (if applicable)
  • * If hospitalized in public hospitals in Hong Kong, copy of Discharge Summary with diagnosis issued by hospital is required. This could replace the Claim Form Part II (Doctor Statement) whenever possible.
  • # If the Claim Form Part II was not filled, the claim document (e.g. receipts) must be written with diagnosis, treatment, operation, if relevant.
  • If the claim has been processed by other insurer(s), please provide the Certified true copy of the receipt and the Settlement Notice(s) / Payment Advice(s) issued by the earlier insurers.
  • If the claim document is neither in Chinese nor English, kindly request the doctor or medical facility to translate the foreign language in English. It will help the claim adjudication significantly.

Claims Submission

Once the claim has been processed, you will receive another alert email. Then, you may login our Customer Website to check the claim status or view past claim records (up to the past 1 year) under “Group Life & Health > Claim Inquiry” section (only applicable to eligible members).
